PASSPORT SERVICES AND DOCUMENTS REQUIRED

- Original Alien Registration Card or Residence Card(to be shown at the counter) and a copy thereof
- Three (3) recent passport-size photographs (5cm x 5 cm) {Please see Photo Specifications}
-Request letter

 

 

**If there is change in address, please bring any Identification card (Aadhar card,etc.) issued by the government of India or any proof of billing issued by any company owned by the government of India showing the latest address or please see link for the list of documents accepted as proof of address

 

 

**If there is change of name after marriage ;
- Affidavit for change of name after marriage
-Joint photograph
-Request letter for the change of name
-Original Marriage Certificate and a copy thereof.
-Copy of passport of the spouse.

 

 

**If there is change of name due to other reasons;
-Advertisement of the change of name in any leading newspaper at the permanent address in India and at the present address in Japan (original copy of the newspaper should be shown at the counter)

 

 

**For deletion of name of spouse from the passport
-Original and copy of Divorce Decree certified by the Ministry of External Affairs, India if the marriage was solemnized in India or;
-Original and copy of Divorce Decree translated in English and  certified by the Ministry of Foreign Affairs, Japan if the marriage was solemnized in Japan.

 

 

**For inclusion of Spouse’s name on passport
-Original  Marriage Certificate certified by the Ministry of External Affairs, India if the marriage is solemnized in India and a copy thereof or;
-Original Marriage Certificate translated in English and  certified by the Ministry of Foreign Affairs, Japan if the marriage is solemnized in Japan and a copy thereof.

 

 

NEW PASSPORT – FOR MINOR
- Completed application form and Personal Particulars Form with thumb impression (as prescribed in the passport form) or signature of the child
-Child’s current passport and a copy thereof.
- Child’s Alien Registration card or Residence card and copy thereof.
- Three (3) recent passport-size photographs (5cm x 5cm)
- Original and copies of parents passports and Residence cards
-Consent letter signed by both parents (Annexure – H)
-Marriage proof if spouse’s name is not mentioned in either parents’ passport.

PASSPORT FOR NEWBORN CHILDREN AND REGISTRATION OF BIRTH
- Completed application form with thumb impression (as prescribed in the passport form) of the child.
- Child’s Residence Card and a copy thereof. If Resident card is not yet ready, please submit a copy of the receipt of application from the Immigration office.
- Three (3)  passport-size photographs (5cm x 5 cm)
- Originals and copies of parents passports and Resident cards
-Consent letter signed by both parents (Annexure – H)
-Copy of the Birth Certificate from the hospital

CERTIFICATE BASED ON PASSPORT
-Duly filled in and signed Miscellaneous Services Form
- Original passport (to be shown at the counter) and a copy thereof.

-One passport size photograph (5cm x 5cm)
-Application to the Consular Officer on a plain sheet of paper explaining the purpose for which the service is required.

POLICE CLEARANCE CERTIFICATE
- Application on a plain sheet of paper stating the purpose for the request.
- Duly filled in and signed Miscellaneous Services Form and Police Clearance Certificate (PCC) Application Form(please see link)
- 2 photographs (5cm x 5 cm)
- Original passport and a copy thereof.
- Alien Registration card or Residence Card and a copy thereof.
-Certificate of Employment

REPLACEMENT OF LOST / DAMAGED PASSPORT
(a) Ordinary (36 pages)

- Application on a plain sheet to consular officer explaining the circumstances under which the passport was lost / got damaged.
-Completely filled in and signed  Application Form and Personal Particulars Form
- Police report.
- Photocopy (if available) of lost/damaged passport.
- 3 Photographs (5cm x 5 cm).
- Alien Registration card or Residence Card (to be shown at the counter) and a copy thereof .

EMERGENCY CERTIFICATE
- Application form along with passport (even if expired) and a copy thereof.
- If original passport is not available, supporting evidence of Indian nationality like passport copy / driving license with a copy / alien registration card with a copy etc. should be submitted.
- Documents issued by the Immigration Authority, if any.
- 5 Photographs  (5cm x 5 cm)
-Request letter explaining the circumstances warranting issuance of Emergency Certificate.

IDENTITY CERTIFICATE
- Application on a plain sheet of paper explaining reasons for applying.
- Identity Certificate (IC) (Original).
- Photocopy of the IC.
- 5 photographs (5cm x 5 cm)
- Completely filled in and signed Prescribed form
- Alien Registration card or Residence Card(to be shown at the counter) and a copy thereof.
-Request Letter from the HHDL’s office.

**For change in name, change of address, inclusion of spouse’s name, deletion of spouse name or any change of information on passport, the applicant has to submit fresh passport application along with the required supporting documents.
The above mentioned documents are general mandatory requirements.  The applicants may be requested to submit some more documents after scrutiny of the application at the counter, on case to case basis.

 

 

NOTE: The Mode of Payment for applicants seeking Passport, Consular and other services from the Republic of the Marshall Islands is through ‘Bank Transfer’ in Japanese Yen only  and the amount has to be remitted to  the Embassy of India Account Number  424123301000004 at Bank of India, Tokyo Branch;  SWIFT  CODE – BKIDJPJT

In addition to normal processing time of 4-6 weeks for applicants residing in Japan, it may take 10-12 days for the delivery of the passport by Registered Post at Marshall Islands, after its dispatch.
